Care Coordinator Team

The Care Coordination team is provided by the Primary Care Network (PCN), to support patients with accessing services to help with their long term conditions.

Whilst not clinical, they support and help coordinate care around the person to provide advice and guidance in accessing help from other external services, ensuring personalised care plans are in place, referring to specialist services like Diabetes UK and Frailty.

We provide services to support with:

  • Cancer Support Services
  • Child and Young Persons Services
  • COPD and Asthma Services
  • Diabetes Programmes
  • Frailty Services
  • Health and Wellbeing Support Services
  • Weight Management Programmes
